d.Place of birth 要求填City or town,我出生在县,公证书上填的也是XX county,XX province,我应如何填写这栏
e.我已婚尚无子女,14项Dependant1-3我应填N/A还是不填,还是划去这一栏?
f.我爱人自学考试大专学历,她的教育程度填哪一项?
g.所附照片被背面是否写姓名,是否只有主申请人的0008才附照片

l.我从大使馆获得的文件中除了CHECKLIST外,还有REMINDER,SELF-ASSESSMENT,COST表,上面写着填好后与申请一起递回,我现在用计算机填表,如何处理它们?如果递回它们,是否与CHECKLIST一起放在APPLICATION KITS的前面

It's ok, no matter capitalized or uppercases.

You'd better capitalized the address. Use the format that can be understood by both the visa officials and the post man.

Uppercase is ok.

"XX Province" is ok.

Fill in the info of your spouse.
for other dependants, fill with " N/A".

You may fill them by pen. Not necessarily use computer.

When I prepared mine, -sailor(Sailor); 2000-6-6 {467} (#1328@0)
I went to the CIC web site and download all the related PDF files.

It seems there're still that kinds of files there for downloading.

In the download page for independant imm, (url as following)
http://www.cic.gc.ca/english/coming/eindepen.html
You'll see the Guide, Kit for applicants, which includes 3 sections.
So you should download all the applicable files.

All those files are enough for the application. Just following the
instructions inside.
