Yes, you got the same package as mine. You must sign your name in Agreement Form. That means you must agree to support your wife living here for at least 10 years without government's succor.

I guess the duplicated form is for error typing, if you unfortunately fill out the form wrong, you may have one more chance to fill out another one.
As i know, only your wife need to sign on the application form( 4 leaf).
