    大家好！我是DIY，现有几个问题请教大家！
    1）听说办公证需单位出示介绍信，可否不惊动原单位呢？
    2）推荐信可否自己签名？
    3）申请表上的mailing address和living address是不是必需相同呢？问题最大的是怕收信不安全，请问成都是否有邮箱出租？
    4）出生地在重庆，但现在工作在成都，无犯罪证明是否要在两地都开？
    5）不知乙肝大三阳能否通过体检？有谁知道成都指定体检医院的电话和地址？
    6）如果是集体户口怎么班？

      1) some case need, some no need, for example: non-criminal record deed and working experience deed are need, born and (un)married is no need. but there has some way to avoid.
      2) can't, should be signed by your recommender
      3) can be different, otherwise, why they ask you 2 address?, don't know the rent info of mailbox in Chengdu, I don't live there.
      4) must submit info from 18 years old wherever you lived.
      5) not sure, seems can pass, don't worry
      6)same as single, I think
