I, have read the document prepared by the Ontario Ministry of Labour entitled “Information for Employees about Hours of Work and Overtime.”
From time to time, I agree to work hours in excess of 8.0 hours per day, up to 12.0 hours in a day, and/or in excess of 48.0 hours per week, up to 60.0 hours in a week.
On one day in each workweek, I agree that the period that is free from performing work may be shorter than 11 consecutive hours but shall be at least eight consecutive hours.
This does not affect the Company's overtime policies in any way.
This agreement comes into effect the day that it is signed.
I understand that I may revoke this agreement by providing two weeks written notice. I further understand that F&P may also revoke this agreement by providing reasonable notice.
This supersedes any previous agreement regarding excess hours that I have signed.
