Just doctor's signature or seal is OK. Maybe even this is unnecessary. Acutally I just give the family doctor a copy, and fill in day care's form with reference to the English records. When I asked the health ministry staff about the missing immunization, a lady just told me what my baby need and said she will input these records into their computer.
