NOW THEREFORE WITNESSETH in consideration of the services rendered by MY_NAME to COMPANY_NAME, the payment to MY_NAME the sum of two ($2.00) dollars, and other good and valuable consideration, the receipt of which is hereby acknowledged, the Parties hereto agree as follows:
