You get it complete. One item needs clarification, however, as it's not in line with my understanding and experience, actually.

You are right in that a driver must report the collision to police to meet "injury and/or damage" requirment. It's the understanding, however, once s/he so does, her or his insurer will get a report from police automatically, regardless her or his claim against the insuer. And every licensed auto insurer has access to this report ot record as well. So if the driver's intent is to avoid rating incease upon renewal, s/he has to poise to make proper decision before reporting.
