Print one from site below and sign it by you and your tenants.

6. Each year bank will send you a summary statment of your mortgage,
which include such as principle you owe, how much interests you have paid .... etc. if you do not have one, you are better to go to your bank. They should know what kind of paper they need to provide to you.
