I did mine in Jan. You fill out the form on the line. You can put your name and your families' names on the same form. You give your credit card no. at the bottom of the form and you get charged immediately, 6 bucks or something.

In two weeks, you will receive the confirmation letter of this reservation from the US consulate and you are supposed to bring this letter with you when you go for your interview.
